Privacy

We will do everything we can to protect your privacy.

Before we collect, use or share your personal health information, we must get your consent.  With your consent, we will only collect information about you that is necessary to do a good job helping you. You have the right to know why such information is being collected. You do not have to consent to the release of any information about you if you do not want to. You may also withdraw your consent at any time.

Everything you discuss in your assessment or treatment sessions with us is confidential.  Your reports, letters and conversations are private and will not be released to anyone without your consent.  There are some exceptions to this rule, however, such as in situations where a child is in danger.  These exceptions are described on a separate Helpful Information form, which can be made available to you at any time.

We will make every effort to ensure that all information about you is accurate and complete.  You may see your file at any time and have any errors corrected.  We will be happy to help you with this.

We will take proper steps to safeguard your personal health records from theft and unauthorized access, modification or damage.  Your records will be stored (and disposed of, if necessary) in a secure manner.  We will notify you if any problems or breaches occur.

If you have any concerns, whatsoever, please contact Dr. Michael MacDonald at 519-719-4554.  Dr. MacDonald is our information officer.  His job is to protect your privacy and follow the guidelines set out by PHIPA.  PHIPA stands for the Personal Health Information and Protection Act (Bill 31), created by the Ontario government to protect your health care information.
